Understanding the Tandem Stack: Mechanism of Action
Semaglutide (GLP-1SG) is a long-acting analogue of glucagon-like peptide-1. Its design — albumin-binding fatty-acid side chain and dipeptidyl-peptidase-4 protection — was described in the discovery literature[1]. In randomised clinical trials it produced substantial weight reduction in adults with overweight or obesity (STEP 1)[2] and improved glycaemic control in type 2 diabetes (SUSTAIN 1)[3].
Cagrilintide is a long-acting amylin analogue: it engages amylin and calcitonin receptors, a satiety pathway distinct from the incretin axis. Its medicinal-chemistry development was described in 2021[4], and once-weekly cagrilintide was evaluated for weight management in a randomised, placebo- and active-controlled phase 2 trial[5].
What makes this pairing unusual in research catalogues: the combination itself has been studied. Co-administered cagrilintide and semaglutide were evaluated in a randomised clinical trial in adults with overweight or obesity[6], making the amylin + GLP-1 duo a documented model of dual-pathway satiety research.
